What to expect once I have made a referral
Once a referral has been made, our intake specialist will send out a text message with information on how to complete our new patient registration. Once registration is completed another text will be sent out with the link to complete new patient paperwork, where paperwork can be completed easily online. At any time during this process new patients may call in or come in in person for assistance and our team is happy to assist. (Hard copies of our intake paperwork is available in the office.) Once the initial paperwork is finished, our intake team will follow up, either by phone or text, to offer available appointment times and ensure clients feel supported and prepared for their first visit.
